1. I understand that this is a virtual consultation and no physical examination will be performed. I acknowledge and accept the limitations of teleconsultation.

2. I understand that teleconsultation does not replace an in-person medical examination, and the doctor may recommend a physical consultation or further medical tests if required.

3. I understand that this service is not intended for medical emergencies. In case of an emergency, I will seek immediate medical attention at the nearest hospital or emergency facility.

4. I confirm that the information provided by me is accurate and complete, and I consent to sharing my medical information with the doctor for the purpose of consultation.

5. I have read and understood the above terms and consent to proceed with the teleconsultation.
